Somatic and body-based approaches often assume that more awareness of bodily sensation is inherently therapeutic. The research doesn’t fully support that.
For some clients, turning attention inward brings regulation and insight.
For others, it increases hyperarousal, dissociation, and a plethora of symptoms, and that risk doesn’t sort neatly by diagnosis. An anxious client and a depressed client can both go the wrong way with the same well-intentioned body-scan instruction, for opposite reasons.
The more useful question isn’t how much a client notices their body. It’s how they relate to what they notice, as trustworthy information, or as a threat to be managed.

THE FIRST LAyer - The Framework
The Multidimensional Assessment of Interoceptive Awareness (MAIA) was developed in 2012 by Dr Wolf Mehling and colleagues and updated in 2018 to strengthen its reliability. It is now a 37-item, 8-scale measure and one of the most widely used tools for interoceptive awareness in the research literature.
Interoception International Co-Founder and Lead Faculty, Tracy Jarvis, MSc, CSP, UKCP, translates its eight scales into a clinical map, not a questionnaire to score, but eight questions to hold in mind as you listen to any client, regardless of presentation:
- Noticing – can they detect sensation in the body at all? Or how do they do this?
- Not distracting – do they push discomfort away, or stay with it?
- Not worrying – does a sensation stay a sensation, or spiral into catastrophe?
- Attention regulation – can they direct and hold attention on the body on purpose?
- Emotional regulation – can they use bodily cues to track what they’re feeling?
- Self-regulation – does turning toward the body actually produce calm, or more distress?
- Body listening – do they use the body’s information to guide decisions?
- Trusting – is the body a safe place to live, or something to be endured?
The Second Layer - The SENSE Framework™
Not every client’s disrupted relationship to the body looks the same, and it doesn’t map cleanly onto diagnosis. Interoception International’s SENSE Framework™ identifies a client’s predominant interoceptive styles, so you can match your intervention to the pattern in front of you rather than to the label on the intake form.
Clients don’t sit in one category permanently, and they don’t sit in one category by diagnosis. A client can be scanning in the waiting room and numbing twenty minutes into describing what’s actually wrong. This training teaches you to track that movement in real time, and to intervene accordingly.
