A multi-dimensional model of interoception - a framework for therapists
In this fireside conversation with world-renowned researcher Professor Sarah Garfinkel and trauma specialist and lead faculty of Interoception International, Tracy Jarvis, MSc, CSP, UKCP, you’ll hear how interoceptive awareness isn’t one skill. Garfinkel’s research shows that interoception spans multiple dissociable dimensions – detection, attention, and higher-order interpretation, that can each be disrupted independently and don’t always move together. That mismatch shows up differently across trauma, anxiety, autism, and other diagnoses, sometimes as flooded signals a client can’t trust, sometimes as confidence without accurate detection. Understanding which dimension is disrupted, and for which client, is what turns interoception from theory into a tool that’s helpful for your clinical practice.

Professor Antonio Damasio’s work on homeostasis and somatic markers reshaped how we understand feeling itself, not as an add-on to cognition, but as the body’s running commentary on survival. In complex trauma, that commentary breaks down unevenly, disrupting body ownership, agency, and trust in internal signals. This session introduces interoception as a clinical lens, tracing the path from homeostatic regulation to the distinct dimensions of interoceptive processing, and what happens when a client’s felt sense of owning their body is compromised.

Trauma does not only change what people think or feel; it can change what the nervous system is able to access. This presentation explores interoception as the brain’s inside-out sensing and meaning-making system and examines why a person may cognitively know they are safe, connected, or capable while those states remain difficult to feel in the body. Participants will learn how the Interoceptive NeuroEmpowerment Scale can be used to track the moment-to-moment accessibility of adaptive states, guide clinical pacing, and support the development of safety, self-trust, and choice without pressuring clients to produce a preferred response.

Paul Dallaghan, PhD brings decades of breathwork and contemplative practice into direct conversation with interoceptive neuroscience, illuminating how conscious control of respiration shapes afferent signaling from viscera to insula. His session examines breathwork not as adjunct relaxation technique but as a direct lever on interoceptive accuracy, prediction error, and autonomic regulation, offering clinicians a physiological rationale for practices long used intuitively. Participants will leave with a framework for integrating breath-based interventions into interoceptive treatment protocols, grounded in both contemplative lineage and current predictive-coding models of the body.

In this session, Courtney Rolfe, LCPC, uses the research on awe, interoception, and body-based therapy to explore the use of expansive states and resourcing experiences in the therapy room. Awe is more than a nice feeling. It’s a distinct emotional state that shifts how the nervous system responds to stress and connection. Interoception, our felt sense of what’s happening inside the body, shapes how available those expansive moments actually are to a client in session. Body-based approaches to trauma give us practical ways to build that capacity so clients can widen their capacity, and access resourcing states safely, before and alongside deeper processing work. Participants in this traning will leave with an understanding of some of the key research related to expansive states and interoception, as well as practices and techniques that can be immediately brought into the therapy room

The interpreting body in complex trauma: from the anterior insula to the eight pathways of interoceptive awareness in clinical practice
A.D. Craig’s mapping of interoceptive pathways to the anterior insula reframed feeling as an act of interpretation, not mere transmission: the brain building a felt sense of the body’s state moment by moment. This session moves from that neuroscience into Mehling’s MAIA-2 framework, unpacking the eight pathways of interoceptive awareness as a clinical map for how trauma reshapes a client’s relationship to bodily sensation. We examine how disrupted interpretation shows up differently across presentations, from dissociative numbing to hypervigilant overwhelm, and where recovery of embodied trust becomes possible.

Functional neurological disorder (FND) sits at the fault line between neurology and psychiatry, often dismissed as “unexplained” precisely because it doesn’t fit either lens cleanly. Patrick Scott Welsh brings a perspective few clinicians can offer: he has lived FND from the inside, not just studied it. Drawing on allostatic load theory, this session reframes FND symptoms as the body’s overwhelmed regulatory system searching for equilibrium, not malingering or mystery. Patrick traces his own path through diagnosis, disbelief, and recovery, showing clinicians what interoceptive disruption actually feels like from within, and what genuinely helped restore function

Physiological hyperarousal is a common symptom of many mental health disorders. Use of wearable technologies to objectively monitor arousal during and in-between sessions can provide insights to clinicians on the effectiveness and individualization of the therapeutic approach, accelerating successful outcomes. Dr. Melis Yilmaz Balban, a Harvard trained Neuroscientist, will provide an overview the neurobiology of hyperarousal and the data behind the effectiveness and limitations of existing biofeedback tools used to reduce arousal symptoms. Dr. Balban will present a new technology that allows real-time nervous system monitoring in the clinic by the clinicians and its applications in the treatment of broad range of mental health conditions.
